For medical students and residents who, after considering the evidence and their institutional context, determine that modafinil use is appropriate for their situation, several evidence-based principles are worth internalizing.
First, obtain the medication through legitimate channels. A consultation with a physician — including a student health provider — for evaluation of potential shift work sleep disorder or fatigue-related symptoms is both medically appropriate and professionally protective. Self-prescribing or diverting a colleague's prescription carries legal and licensure risks that are disproportionate to any cognitive benefit.
Second, recognize the interaction between modafinil and sleep debt. The drug attenuates the subjective experience of fatigue more effectively than it restores the underlying neurological deficits associated with sleep loss.
